Across Southeast Asia, healthcare demand is expanding rapidly. Population growth, urbanisation and rising incomes are driving increased demand for specialist care, while national health systems across the region are under pressure to expand capacity. Yet despite significant progress, access to advanced specialist services remains uneven across many Southeast Asian markets. Outside of a few major healthcare hubs such as Singapore, Bangkok and Kuala Lumpur, patients in many cities still face long waiting times or must travel abroad for treatment in areas such as oncology, cardiology, neurology and advanced diagnostics.

For decades, this gap has fuelled medical tourism in the region. However, the next phase of regional healthcare development may look different. Rather than relying solely on cross-border patient travel, many countries are now focused on building stronger domestic specialist capabilities. This is creating new opportunities for regional collaboration between healthcare providers, investors and experienced specialists.

Indonesia illustrates both the scale of demand and the structural gaps that remain across the region. With a population exceeding 270 million people, Indonesia faces a rapidly rising burden of non-communicable diseases, including cancer. At the same time, access to advanced oncology services remains concentrated in a relatively small number of major hospitals. Comprehensive cancer care requires multidisciplinary teams, specialised facilities and advanced equipment such as PET/CT, radiotherapy systems and integrated oncology diagnostics. These capabilities are still largely concentrated in Jakarta and a handful of major cities. Access to PET/CT remains limited nationwide; the Indonesian government has acknowledged this gap and plans to expand PET/CT capacity to approximately 21 units across 16 hospitals by 2027.

This uneven distribution creates significant access challenges. Patients in secondary cities often travel long distances to access specialist diagnosis and treatment, or seek care overseas. These patterns are reflected across much of Southeast Asia. In many countries, the growth in cancer incidence and other complex diseases is outpacing the development of specialist infrastructure.

A shift toward regional care ecosystems

Historically, the response to these gaps has been medical tourism — patients travelling to established centres such as Singapore for complex care. However, healthcare systems across the region are increasingly looking to develop local specialist capabilities while maintaining access to international expertise. This is leading to the emergence of what can be described as regional care ecosystems, where specialist expertise, diagnostics and treatment platforms are developed closer to where patients live through cross-border collaboration.

Several models may begin to emerge.

Specialist centres developed within hospital networks. Private hospital groups across Southeast Asia are increasingly exploring dedicated specialist platforms, including cancer centres, cardiac centres and advanced diagnostic hubs.

Regional referral networks. Complex cases may continue to be referred to Singapore for treatment, while diagnostics, follow-up care and selected procedures take place locally.

Visiting specialist and collaborative clinical programmes. Singapore specialists may participate in regional programmes through visiting practice arrangements or structured clinical collaborations.

These models allow healthcare systems to expand access to advanced care while leveraging established expertise. Singapore has developed a strong reputation as a regional centre for high-quality specialist medicine. Many Singapore specialists have extensive experience managing complex cases, multidisciplinary care and advanced diagnostic platforms. Training standards and clinical governance frameworks in Singapore are widely recognised across the region. As healthcare systems in Southeast Asia evolve, this expertise is increasingly valuable.

For specialist clinics in Singapore, regional collaboration may offer several strategic advantages: participation in fast-growing healthcare markets; opportunities to expand clinical reach beyond a single domestic market; development of regional referral networks; and collaboration with hospital groups and healthcare investors.

Importantly, participation does not always require building a full overseas clinic from the outset. Many regional healthcare platforms begin with partnership-based models, allowing specialists to extend their expertise while maintaining their core clinical practice in Singapore.

From opportunity to execution

While demand for specialist care is clear, successful healthcare expansion depends on more than market size alone. Experience across regional projects suggests several factors are critical: a strong referral ecosystem linking hospitals, specialists and diagnostics; alignment with local healthcare providers and hospital partners; clear regulatory pathways and licensing frameworks; and appropriate capital structuring for healthcare assets.

Without these foundations, even markets with strong demand can be difficult to develop sustainably.

As Southeast Asia continues to expand its healthcare infrastructure, the role of cross-border specialist collaboration is likely to grow. For Singapore specialists, the question is no longer simply whether demand exists in the region — but how to participate in building the next generation of specialist care platforms. Those who are able to combine clinical excellence with the right partnerships, infrastructure and regional networks are likely to play an important role in shaping the future healthcare landscape of Southeast Asia.
